MSK Pathway Information

Target Patients:
A patient will be considered eligible for referral to the service if they present with a condition thought primarily to be of MSK origin, including but not limited to orthopaedics, MSK-related pain and back/spine.


The following patients are excluded from Musculoskeletal Clinical Assessment and Treatment Service (MSK CATS) (these are not applied to the outpatient physiotherapy service):
• Children (under 18 years)
• Patients who are not registered with a Derbyshire/Derby City GP Practice 
• Immediate life-threatening conditions
• Acute trauma
• Suspected 'red flag' patients (e.g. cauda-equina, systematically unwell, significant weight loss suggestive of serious issue)

Which pathway should I follow with my musculoskeletal patient?

Patients should be referred to the relevant service for their condition highlighted below:

Podiatry 

  • Toe problems
  • Foot problems
  • Heel pain
  • Persistent ankle pain
  • Arthritis of the foot or ankle
  • Achilles Tendon problems
  • Structural and biomechanical problems of the foot and lower limb 

Physiotherapy

  • Back pain including sciatica
  • Neck pain including radiculopathy into the arm
  • Shoulder pain/trauma
  • Elbow pain
  • Hand/wrist pain
  • Hip pain
  • Knee pain
  • Ankle pain/sprain
  • Muscle and tendon injuries
  • Chronic musculoskeletal pain 

Patients seen within this service can also have access to a robust escalation process to senior clinicians who can inject, refer on to secondary care and request diagnostics. 

Service provided by DCHS & CRH also includes intra-articular and peri-articular injection therapy.

MSK Triage Service

  • Patients who have completed a course of self-management plus physiotherapy, have not improved and require further management; including referral to secondary care.
  • Patients who have completed a course of self-management plus podiatry for musculoskeletal problems, have not improved and require further management; including referral to secondary care.
  • Patients who may need referral to secondary care for injections under imaging (demonstrating that the PLCV/NICE guidelines have been followed) 

Patients should not be referred to the MSK CATS if:

  • They have not tried physiotherapy and self-management (unless the situation dictates that they were not appropriate measures to undertake prior to referral)
  • They are already receiving physiotherapy services
  • They require a visit to their home
  • They present with red flags or suspected fracture

Orthopaedic on Call

Derby & Burton: 
The GP will call the Bed Bureau on 01332 789099 to arrange any admission. They take the details and contact the relevant admitting doctor regardless of which speciality it is.

Chesterfield:
Semi-urgent patients would be admitted to the surgical admissions unit, with the GP having let the orthopaedic team know by bleeping the SHO on 982 and additionally the registrar on 983.

GPs can refer patients directly to Derby and Chesterfield Hospitals. 
1) Clearly highlight that the referral is for the Fracture Clinic (This allows patients to be paper triaged and allocated to the most appropriate clinics, i.e. foot/ankle, spinal fractures)
2) Include as much clinical detail as possible
3) Email to the appropriate generic email address

Outpatient Physiotherapy, Occupational Therapy and MSK Triage Service

The DCHS Outpatient Physiotherapy, Occupational Therapy and MSK Triage service have a new website where people can be directed for advice and self-management information on common MSK conditions.
The website also includes an online self referral form should people wish to access outpatient physiotherapy and occupational therapy services in this way.
People can continue to self-refer via the Therapy Direct telephone line:
01246 565050 (this is a change of number for some areas).
Phone line open from 8.30am - 4.30pm Monday to Friday.
Please note that self-referral is only available for people aged 18 years and over. A GP referral via eRS is still required for people requiring the MSK Triage service.

ESCAPE-pain
The ESCAPE-pain programme aims to increase physical function and improve quality of life. ESCAPE-pain is a rehabilitation programme for people with chronic joint pain that integrates educational self-management and coping strategies with an exercise regimen individualised for each participant. It helps people understand their condition, teaches them simple things they can help themselves with, and takes them through a progressive exercise programme so they learn how to cope with pain better.
