Referral Information

If the patient is diagnosed with Type 2 diabetes (48mmol/mol or above), refer to Diabetes Type 2 Education X-PERT/Diabetes & You within Structured Education Referrals.

If the patient is pre-diabetic (42-47mmol/mol), refer to Healthier You within Structured Education Referrals.

If patient does not wish to access Diabetes Structured Education Services or requests Weight Management Referral, consider the below:

BMI 25+ (BMI 23+ for Black and Asian ethnicity): refer to Live Life Better Derbyshire.

BMI ≥30 (adjusted to 27.5 for South Asian ethnicities): refer to Livewell.

BMI of 30+ (adjusted to ≥27.5 for people from Black, Asian and ethnic minority backgrounds) AND has a diagnosis of diabetes (Type 1 or Type 2) or hypertension or both: refer to Digital Weight Management.

Live Life Better Derbyshire offers 12 weeks of free physical activity advice and support to people who are on our weight management programme and for people who are inactive (completing less than 30 minutes of moderate activity per week).

Whatever your patients starting point, Live Life Better Derbyshire has plenty of ideas and advice to help your patients get started and move more.

Live Life Better Derbyshire provides support to clients aged 18 and over, living in Derbyshire and or registered with a Derbyshire GP.

What Live Life Better Derbyshire offers:

  • A friendly, professional one to one telephone support service
  • Support in accessing existing community based activities 
  • Support the client in finding activities that work for them such as home exercises or chair based exercise
  • Support the client with increasing confidence to move more 
  • Face to face community physical activity sessions - clinics page. 
  • Physical activity virtual sessions (delivered over the internet via a compatible computer or phone)
  • A holistic health and wellbeing MOT

For under 18s, Active Derbyshire is a great tool for finding appropriate activities for younger people. Each locality is slightly different in what they offer as they hold the funding for young people's physical activity.

Digital Weight Management

Eligibility Criteria

The NHS Digital Weight Management Programme is offered to adults with a diagnosis of diabetes (Type 2 or Type 1) and/ or hypertension with a BMI of 30+ kg/m2 (adjusted appropriately for ethnicity).

The programme will only be accessible to those who have a smartphone or computer with internet access. You will need to have an e-mail account to access the programme and get the full benefits. This programme is delivered exclusively digitally.

However, people living with obesity will continue to be able to access Local Authority commissioned weight management services where the new digital services offered do not meet their needs.
