Same Day Emergency Care (SDEC)

SDEC builds on ambulatory care by providing rapid assessment, diagnosis and treatment to patients who need emergency care but don't necessarily need to be admitted to hospital. These patients can be streamed through a hospital ED, transferred by ambulance or referred via NHS11 or their GP and is designed to reduce the number of unnecessary admissions, reduce the pressure on NHS services and improve the patient experience.
This leaflet explains more about how to contact the SDEC for referrals, when to refer and what times the service operates.

Inclusion Criteria:
- Registered with a Derbyshire GP, AND
- Age 16 and above, AND
- Has capacity to give consent OR a "best interests" decision has been made on their behalf, AND
- Has dexterity to use the devices OR has carer/NOK who has dexterity to use the devices (please note this does not include the smart phone as Customer Service agents can control this remotely), AND
- An acutely unwell patient who can be managed, monitored and treated at home, AND
- Has no oxygen requirements, AND
- Where objective and subjective monitoring could provide support to avoid emergency admission to hospital
Exclusion Criteria:
- Requires emergency care interventions/has treatment escalation needs, necessitating acute/complex diagnostics and/or clinical interventions that can only be offered in secondary care (and these are appropriate for and in the best interest of the patient)
- Lives in an area with no telecom signal AND Wi-Fi AND doesn't have a telephone landline. Only if a patient does not have access to any one of these options is this a reason for exclusion
- Patients who are not resident in the area.
- Pregnant with oxygen saturations of <94%
