Compass: Changing Lives

Service Information: Covers patients aged 5 to 18 years (up to age 25 if SEND or care leaver), who will benefit from a short term intervention.

They can assist with:

  • Low mood: sadness, low motivation
  • Mild to moderate anxiety: worries, irrational fears, and concerns
  • Common challenging behaviours; angry outbursts, pushing boundaries, frustration, and distress
  • Family and peer relationship difficulties
  • Difficulty adjusting to change and transition
  • Difficulty managing emotions

Compass Changing Lives cannot work with children and young people who:

  • Are currently engaging with any other emotional-wellbeing service
  • Have a diagnosis of ‘clinical’ depression, severe anxiety, Obsessive-Compulsive Disorder (OCD), schizophrenia, eating disorders, psychosis.
  • Have self-harmed long term and currently experiencing significant suicidal thoughts/behaviours.
  • Are requiring long-term therapy.
  • Are in crisis or requiring out of hours support.

In Derbyshire, public health nurses for children and young people are part of the Derbyshire Family Health Service and Derbyshire Healthcare NHS Foundation Trust. They provide services for children from birth to 19, including school nursing and health visiting, teen health & wellbeing, immunisation services.

DFHS provides health visiting services for babies and young children, including developmental reviews, immunizations, and parenting support.

School Nursing (5-19 years):

DFHS also includes school nurses who work in schools, providing support and information to young people on various health topics, including sexual health, drug and alcohol use, and healthy lifestyles.

Our Thrive Social Prescribing Wellbeing Mentors work 1:1 with young people (14-18) and young adults (19-25) to discover what social issues are affecting their health and wellbeing, centring on a holistic approach that identifies their individual needs.

Children in Care
JUCD: Children in Care

In Chesterfield, support for children in care is provided through a combination of health services and social care services. Health services, like the Children in Care Health Team at Chesterfield Royal Hospital and the Derbyshire Children's Community Nursing Team, focus on assessing and meeting the health needs of children in care and their families. Social care services, like those offered by Derbyshire County Council, provide support with placement, welfare, and advocacy for children in care.

Adopted Children
Adoption Support

For children who are adopted, any post adoption support needs are provided for the 1st three years by the child's placing local authority. After this, for adopters living in Derbyshire they need to contact Adoption East Midlands for issues relating to post-adoption support and therapy needs.
