Healthier Together

CATCH
CATCH (Common Approach to Children’s Health) is an award-winning health app for parents and carers of children aged 0-5. Commissioned by NHS and Public Health teams for a particular area, content in the app is professionally and clinically approved, so you can be sure you’re getting the right advice.

Baby Check (Lullaby Trust)
The Baby Check app has been designed to help you decide whether your baby needs to see a health professional. The app will ask you to check your baby for 17 signs and symptoms and then give you advice as to what you should do next. Spotting early when a baby is very ill early can help in their treatment and recovery.

NHS Child Health
Looking after your child’s health, an NHS guide for parents and carers of children aged 0-5 years. Developed by hospital specialists, doctors, health visitors and pharmacists this app will give you lots of useful hints and tips from experts in child health from everything from oral health, upset tummies and diarrhoea to advice on bumps and bruises.

HANDi
HANDi has been developed by the paediatric team at Musgrove Park Hospital to provide expert support to parents/carers and medical professionals looking after children with the most common childhood illnesses

Who Do You Want To Refer To?

  • Physiotherapy Assessment Only 
    • This team provides assessment and intervention for children with developmental and acquired movement difficulties. If unsure, please contact the service for advice:
      0300 123 4586.
      Please complete additional Physiotherapy referral details on form.
  • Occupational Therapy Assessment Only
    • This team provides assessment and intervention for children with functional difficulties affecting participation in daily life activities due to physical or perceptual difficulties (referrals for children with behavioural difficulties are not accepted). If unsure, please contact the service for advice:
      0300 123 4586.
  • Joint Occupational Therapy and Physiotherapy Assessment 
    • The two teams work together where mobility and movement skills are impacting on every-day life. Usually a referral to the Early Years team would be for both services. If unsure, please contact the service for advice:

Children's Physiotherapy Service (CRH)

To make a referral to children's physiotherapy, please complete all sections of the referral form and review referral criteria in checklist 1 and/or 2 as indicated. Please note, the service selected on this form will determine where this referral is directed.

Nottingham: 
Breastfeeding babies - Referral > Paediatric surgeons @ QMC via choose and book for a breastfeeding TT appointment. 
Bottle feeding babies - Referral > Paediatric surgeons @ QMC via choose and book however it would be vetted & appts would be in general clinic. 

Derby:
Refer to maxillofacial dept stating the severity and issues associated with the TT.
Midwives & Health Visitors can now refer directly to RDH rather than via GP, however this is a new service, of which not all staff may be aware.

Child Health and School Nursing
Derbyshire Family Health Service

School nurses no longer accept tasks and instead require an official referral form via Derbyshire Family Health Service.

The below information can be sent to the parent/carer or child to self-refer:
School nurses visit schools across Derbyshire and can support you with any aspects of your health and wellbeing.
