Service Information
Treetops Hospice

Only practices in Derby, Erewash, and South Derbyshire can refer to Treetops Hospice.
Treetops Hospice provides the following services:

Treetops Hospice Services
Therapeutic Services

Therapeutic Services provide counselling and complementary therapy for patients suffering from Bereavement and Life Limiting Illnesses. Therapeutics will only accept self-referrals. 

All services are provided free of charge, and are not means-tested. Patients should please phone Therapeutic Services during our opening hours (8.30am-4pm Monday-Thursday, 8.30am-3.30pm Friday):

We will arrange an assessment with you, at which the most appropriate form of support for you can be discussed.

Befriending

The Befriending Service offers social connection so that those who have a life limiting illness feel more connected to their community at a time when they might otherwise be lonely or isolated.

To be eligible for a Befriending referral the patient has to be over 18 and suffering from a life limiting illness.

Wellbeing Space

The Wellbeing Space at Risley runs a series of different services aimed at supporting people living with a life limiting condition and maintaining independence.  This includes a support and information café, a maintaining independence wellbeing programme (SPRING), exercise classes, thai chi and more – all for patients over 18 with a life limiting illness.
The Wellbeing Space also runs a peer support group for Bereavement.

Referral Process:

Health and social care professionals can refer patients by phoning the Wellbeing at Home Team:

Hospice at Home

There are two main services available, Treetops Hospice at Home and the Treetops Hospice Roaming Service.
Hospice at Home: Our team of qualified Nurses and experienced Health Care Assistants can offer support overnight in the patient's home.   Overnight stays allow family/carers chance to have a good night's sleep, safe in the knowledge that one of our skilled team is present to look after them and provide any care they need.  This service is available from 22:00hrs -07:00hrs. The Hospice at Home Office Coordination Team will contact the patient to discuss availability of this support once a referral has been made.
Treetops Hospice Roaming Service: We recognise that not everyone will want a full night's stay. However, should a patient need any assistance during the night, our Roaming Team (1 x Registered Nurse and 1x Health Care Assistant) will be able to provide the following support: 
•    Personal Care: Mouth care, continence care or pressure care.
•    Assistance with moving and handling of a patient.
•    Assistance with prompting or administering medication. 
•    Symptom management.
•    Patient well-being checks.
This service is available from the hours of 21.30hrs – 06.30hrs. The Hospice at Home Office Coordination Team will contact the patient to discuss access to this support once a referral has been made.
The criteria for both services are as follows:
•    The patient must have a prognosis of six months or less
•    The patient must be known to a District Nursing Team
•    The patient is 18 years or over

Wellbeing at Home


Treetops Wellbeing at Home service provides support in people's own homes during the day. 

People must be:
•             Aged 18 years or over
•             Registered with a Derbyshire or Nottinghamshire General Practitioner
•             Diagnosed with a non-curable and progressive life-limiting illness with palliative care needs

We will assess each person, communicate with them and their loved ones to establish their needs.  

Examples of interventions we may undertake are
Legacy work-oral history, memory box, letters
Future care planning 
Goal setting
Hand/foot massage
Meditation
Chair based exercises
Manicure/pedicure/shave
Additional personal care and assistance (supplementary to provided care)
Assistance with hobbies- meaningful occupation/distraction/symptom management
End of life care.

SPRING

SPRING is a one-to-one bespoke programme which takes place over 12 weeks. We work with patients facing a life-limiting illness, either individually or as a family. It is a holistic programme, where we support them to understand how they are feeling, and incorporating strategies for self-care and seeking sources of support.

The first 7 weeks cover physical, psychological, emotional, social, spiritual, financial, practical and caring needs. We then spend 3-4 weeks working on personal goals e.g. legacy work, mindfulness, creative work, etc.

Referral Process:

• The service is available to anyone with a diagnosis of a life-limiting illness.
• You or your colleagues can refer, or patients can refer themselves.
