CRH Single Point of Contact

A web form for general practice to use can be found here: 

https://www.chesterfieldroyal.nhs.uk/gp-section - Password is crhgp.

It will mainly be used for planned and non-emergency care queries that currently have different access points such as:

•    Clinical queries relating to patient care plans, condition management or medication
•    Chasing test and diagnostic results
•    Waiting list and waiting time queries

The form is for primary care colleagues to use for existing patients and those who have been recently (within 28 days) discharged from Chesterfield Royal Hospital.

It will be received on the webpage and the aim is to have it sent directly onto the relevant speciality or team for a response. We anticipate a response should be available within a week and up to 2 weeks at most.

It is designed to support direct queries and be a single point of contact between primary care and the Trust. It is not designed to remove existing links and connections with colleagues or to support urgent or emergency care, or to receive immediate feedback.

The challenge is to focus on an improvement approach rather than rushing to a ‘top-down’ solution and ensuring the design dovetails with digital transformation e.g. Electronic Patient Record (EPR) and Patient Knows Best (PKB). It will be an iterative process which improves over time. We will communicate the link once it is available. 

DCHS Single Point of Contact

Derbyshire Community Health Services (DCHS) has established a streamlined process to support GP queries that cannot be resolved through existing channels.

To ensure your concerns are directed appropriately, please use the DCHS Patient Experience Team inbox at DCHST.PatientExperienceTeam@nhs.net  as a single point of contact for queries where:

  •  The appropriate service contact is unclear.
  •  It is uncertain which service is responsible.
  •  There are organisational or process issues with broader implications requiring escalation beyond an individual service.

 We hope this simplifies the process and ensures prompt resolution of queries. Please don’t hesitate to get in touch using the provided email address.

DHFT Single Point of Contact

This all relates to adult working age mental health services.


Each Community Mental Health Team (CMHT) will still have its own direct telephone numbers, e-mail inbox for queries and GPs can use the tasking functionality in SystmOne.
In addition, a new single point of access phone line has been set up for primary care to access responsive non-urgent clinical support for mental health related queries.
The Single Point of Contact telephone number is 01332 502111. This number will give the GPs/primary care only, quick direct access to the teams, in addition to the other CMHT numbers they already have.
The number has 3 options:
•    CMHT
•    Crisis team
•    Early Intervention

For CMHT - there is option to be directed to all 'Adult Working Age' Community Mental Health teams.
For Crisis teams - there is an option to be directed to the three Crisis teams.
For Early Intervention - there is an option to be directly to the two Early Intervention teams.
This will be open to general practice staff, including non-clinical staff (not to be shared with patients). 
This will link primary care to the local Community Mental Health Team for their area via options on the phone line and will be answered by the local Duty Worker. 

The Duty Workers have access to consultants and are available Monday-Friday 9-5pm. 
For queries about all other mental health services (including Older Peoples, Children's and Forensic Team etc) please contact those teams directly or ring Derbyshire NHS Healthcare Foundation Trust at Kingsway Hospital switchboard: 01332 623700 who will redirect you to the requested team.

RDH Single Point of Contact

A new phone line, managed by Derbyshire Healthcare United (DHU), is the Single Point of Contact, now open to all Derby City PCNs. Roll out of the Single Point of Contact across other Derbyshire PCNs is planned and will be communicated in due course.

It fields queries from primary care. DHU will take calls from practices, directing them to UHDB specialties, calling patients with responses to queries.
It is open Monday to Friday 9am-5pm. Calls received by practices after 5pm will go to DHU the next morning.
